Knee pain is one of the most common reasons people put off the activities they love — and one of the most common reasons people start researching surgery. But surgery isn't the only option, and for many causes of knee pain, it isn't even the first option your doctor will recommend.
Common Causes of Knee Pain
Knee pain can come from a wide range of sources, including:
- Osteoarthritis and general wear-and-tear of the joint
- Meniscus tears, from acute injuries or gradual degeneration
- Patellofemoral pain (pain around or behind the kneecap)
- Ligament sprains or strains, including the ACL and MCL
- Tendinitis from overuse, especially in runners and jumping athletes
Non-Surgical Options to Try First
Depending on your diagnosis, several conservative approaches can reduce knee pain and improve function without surgery:
- Physical therapy to correct strength deficits and movement patterns that overload the joint

How Physical Therapy Treats Knee Pain
The knee doesn't work in isolation — weakness or tightness in the hips, glutes, and ankles often shows up as knee pain. Your physical therapist will assess the whole kinetic chain, then build a plan that typically includes targeted strengthening of the quadriceps, hamstrings, and hip muscles, gait and movement retraining, and hands-on treatment to address any joint restrictions. The goal is to reduce the load on the painful structure while you rebuild the strength and control that protect the joint long-term.
When Is Knee Surgery the Right Call?
Surgery is sometimes the best option — for example, with certain mechanical meniscus tears that cause locking, significant ligament ruptures in active individuals, or advanced bone-on-bone arthritis that hasn't responded to a genuine course of conservative care. If that describes your situation, physical therapy still has a role to play in preparing your knee for surgery and guiding your recovery afterward.

It depends on the type, size, and location of the tear. Many degenerative meniscus tears, especially in older adults, respond well to physical therapy alone. Tears with mechanical locking or in younger, more active patients may need a surgical opinion.
